Aged Care Assessment Teams – (ACATs)
ACATs help the elderly and those that care for them decide what type or level of care will be most suitable for when they are no longer able to manage at home.  

 

Mainly Government funded, these teams offer information about and referrals to suitable care facilities.

Made up of health professionals, doctors, nurses and social workers the teams assess your needs using a series of questions aimed at creating a picture of what level of assistance you need. 

A range of assistance can be recommended including a Community Aged Care Package, which coordinates different care services, provided to you in your own home.

A need for high or low care services will be determined and an appropriate facility referral offered. 

Facilities include Nursing Homes, Hostels and Retirement Villages. Some offer both high and low care and a wide range of lifestyle services.
